The forms you fill out when you arrive for your first visit will be scanned into your electronic medical record. Additionally, you will use a program called Instant Medical History. Please bring all lab work, EKGs, genetic data, coronary calcium testing reports, and any other paperwork that you feel will help Dr. Eldridge care for you.
